Use of Antibiotics-Impregnated Cement Spacers for the Management of Postoperative Infection Following Anterior Cervical Discectomy and Fusion: A Case Report

Abstract

|
Study Design: Case report Objectives: We report a case of a postoperative infection following anterior cervical discectomy and fusion (ACDF) treated using antibiotics-impregnated cement spacers.
Summary of Literature Review: To the best of our knowledge, there has been no report on the management of postoperative infection following ACDF using cement spacers impregnated with antibiotics.
Materials and Methods: A 57-year-old man visited our institution due to gait disturbance, clumsy hand, and weakness of the right upper extremity. He was diagnosed with cervical spondylotic myeloradiculopathy and underwent C5-6-7 ACDF. Postoperative fever, swelling, and redness at the surgical site were observed, and magnetic resonance imaging revealed an infection of the surgical site extending to the mediastinum. At postoperative 5 days, debridement and insertion of antibiotics-impregnated cement spacers into the disc spaces were carried out. The cement spacers were exchanged 14 days after the first surgery.
Results: When the infection was controlled, we removed the spacers and grafted autogenous iliac bone. There was no recurrence of infection at a 1-year follow-up after the last surgery.
Conclusions: We suggest antibiotics-impregnated cement spacers as a useful treatment option for controlling postoperative infection following ACDF.
